BACKGROUNDPrimary care in the United States is currently in crisis. With misaligned financial incentives and an insufficient number of providers, the system has proven unsatisfactory for both physicians and patients. Additionally, health care reform’s implications for health insurance will result in a large influx of new patients beginning in 2014.
There are many proposed solutions to this crisis. Health Information Technology (HIT) is an area of interest earmarked by the government and frequently cited as a way to increase efficiency and quality of patient care. Additionally, the Patient-Centered Medical Home (PCMH) model has been gaining momentum as the optimal means to enhance patient care. While both of these solutions have potential benefits, there are many barriers to adoption and few proven successful outcomes.
THE FROMER-GPF PATIENT JOURNEYGroup Practice Forum (GPF) leverages deep knowledge of practical physician group dynamics and clinical guidelines to create the FROMER-GPF PATIENT JOURNEY: a proprietary workflow solution designed to provide primary care physicians with simple solutions that can be incorporated into their practices immediately to improve patient care.
